For good and valuable consideration herein acknowledged as received, I hereby grant to VetStem its heirs, legal representatives, and assigns, those for whom Photographer is acting, and those acting with his/her authority and permission (hereinafter collectively referred to as the "Users"), the irrevocable and unrestricted right and permission to take, use, re-use, publish, and republish photographic portraits or pictures of the Model including, without limitation, reproductions thereof in which the Model may be included, in whole or in part (hereinafter collectively referred to as the "Photographs").
The Photographs may be used by VetStem and their licensees, partners, and affiliates for illustration, promotion, art, editorial, advertising, trade, in formats including but not limited to television and motion pictures, internet, magazines, books, video recordings, audio recordings or other media, known or unknown at this time, or any other lawful purpose whatsoever.
The Model hereby waives any right to inspect or approve the finished product or products and the advertising copy or other matter that may be used in conjunction therewith or the use to which it may be applied.
I herby grant VetStem permission to interview me and/or to use my likeness in photograph(s) /video in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by VetStem, in perpetuity, and for other use by VetStem. I will make no monetary or other claim against VetStem for the use of the interview and/or the photograph(s)/video.

I hereby agree to the Terms and Conditions and grant VetStem permission to use all or any portion of my testimonial in any and all of its existing or future publications and in any and all other media. My testimonial may be rewritten for better clarity without further permission required.
